Parish Self-Study Survey

Your pastor and staff request that every parishioner 20 or over fill in and submit the survey below. It is the intention of this survey to provide each respondent the opportunity to do the following:

1. Evaluate the services presently provided by the clergy and staff and, to a lesser extent, evaluate the present use of our physical facilities.

2. Suggest that the services presently provided and the physical facilities used be improved, consolidated, or eliminated

3. Recommend other services that might be provided without increasing existing staff.

Survey forms are being made available in various ways – in the church, on the church’s web site, and by mail. We request only one response per person. We will keep all individual responses confidential, publicly reporting only totals and summaries of the results.

Some entries request written responses. Some request a specific selection or selections. Many request that you select from a range of answers (1 to 5). Sometimes the response requested is “from 1 (least important) to 5 (most important) and sometimes it is “from 1 (strongly disagree) to 5 (strongly agree)”.

We hope you will answer all questions to the best of your ability, but we do understand that you may not have an opinion on some of them and may leave them blank.
